Custom forms & questionnaires

Find the forms your office built, and how each kind of question works on the phone.

Overview

Your office builds its own forms in the Report Designer - inspections, certificates, checklists - and decides where each one appears. You fill them in on one scrolling screen and save, and the office gets the answers and a finished report.

Where forms appear

The office places each form in one or more of these places:

  • The home screen - as a button with the form’s name.
  • A form group on the home screen - a button that opens a list of forms.
  • The job’s Info tab - under Job Checklists/forms.
  • The job’s Create tab - under Checklists.
  • The job’s Close tab - with the stop-work items.

A form on a job can be limited to certain job types, so a form may appear on one job and not another. If a form you expected is missing, ask the office where they placed it.

Kinds of question

On the formWhat you do
A one-line or multi-line boxType your answer
Yes / NoTap one. Some also offer N/A
A number boxType a number. If it has limits, the heading shows the maximum and the form tells you if you go outside them
A calculated valueNothing - it works itself out from other answers as you type
A date or timeTap to pick. Some only allow future, or only past, dates
Options to choose fromTap one option, pick from a dropdown, or tick several - the form shows which. Other opens a box for your own answer
A tick boxTick it
A signature padSign with your finger. Clear signature starts again
Take Photo / Choose ImageOne photo for that question - take a new one, or pick one already on the phone
A table with + AddOne row per item. Tap + Add for another row
A picture or a block of textNothing - it is guidance from the office

Some details fill themselves in, such as the job number, customer and site, or your own name.

Questions that appear as you answer

A form can hold questions back until they are needed - a “why not?” box that only appears when you answer No, for example. If new questions appear as you go, that is the form working as designed.

Required questions

Questions the office has marked as required must be answered before the form will save. The form tells you which ones are missing.

One page

The whole form is one scrolling screen; there are no pages to move between. Long forms are split into headed sections - scroll down to work through them.

Saving

Tap Save when you are done. Some forms also offer Save Draft so you can finish later. See Submitting forms and Save a form and finish it later.
